PATIENT INSURANCE
Insurance information is required before surgery.
Every effort will be made to take care of this in advance. All deductibles and co-pays will be the responsibility of the patient based on their personal insurance plan.
Prior to your surgery date you will receive a phone call in which we will discuss verification of insurance, payment of deductibles, and co-pays, etc.
If anesthesia service is required, these charges will be billed separately by the group covering your case. These charges will come from the anesthesia group with whom your anesthesiologist is a member of.
